People who are developing psychological problems usually are quite aware that something is not quite right in their head... though this piece is quite interesting

Right, however people usually deny the fact because they want to hold on to their sanity.
In fact, that's what this song has in mind, there's bad chords, the voice is altered, and laughing interrupts the word "Psychosis"
